Referrals
It is easy to make a referral to our agency. Referrals may be submitted by physicians or by families who are concerned with their child's development. We have therapists who speak English and Spanish.
For Families and Caregivers
- We will need a physician's order to initiate the evaluation process. Please call our office at 832-484-3756. We will obtain the information from you so that we may assist you with contacting the doctor to request the order. Please have the child's physician and insurance information available.
- If you already have an order from your child's physician and you would like us to evaluate your child, please fax it to 832-324-7860.
- Families may also complete the referral form below
After the signed order is received by our office, our staff will contact you to schedule the initial evaluation.

Communication Essentials accepts referrals from physician's or other health care professionals in many ways:
